PAN form: Key things to remember
- Fill the form in block letters only.
- Use black ink to fill the form.
- Form should be filled in English only.
- Affix a latest colour photograph with white background. The size of the photo should be 3.5cm x 2.5cm. Use adhesive to attach the photo.
- Signature or the left thumb impression should be provided across the photo. The impression should be within the box.
- Thumb impression should be attested by a magistrate of a gazetted office or a public notary.
- It is mandatory to attach identity proof, address proof and proof of date of birth with Form 49A.
