PAN form: Key things to remember

  • Fill the form in block letters only.
  • Use black ink to fill the form.
  • Form should be filled in English only.
  • Affix a latest colour photograph with white background. The size of the photo should be 3.5cm x 2.5cm. Use adhesive to attach the photo.
  • Signature or the left thumb impression should be provided across the photo. The impression should be within the box.
  • Thumb impression should be attested by a magistrate of a gazetted office or a public notary.
  • It is mandatory to attach identity proof, address proof and proof of date of birth with Form 49A.
