- Spouse’s copy of passport

Birth date proof
- Birth certificate
- Life insurance policy bond
- Copy of the applicant’s service record (only for Government employees) or the Pay Pension Order (only for retired Government employees), duly attested/certified by the officer/in-charge of the Administration of the applicant’s concerned Ministry/Department.
- Transfer/School leaving/Matriculation Certificate issued by the school last attended/recognised educational board

Old passport in original with self-attested photocopies of its first and last two pages, including the ECR/Non-ECR page (previously ECNR) and the page of observation (if any), made by the Passport Issuing Authority, and the validity extension page, if any, in the case of a passport with a short validity.
